Background: In the present study, we aimed to better investigate attention system profile of Parkinson's disease-Mild Cognitive Impairment patients and to determine if specific attentional deficits are associated with 123I-FP-CIT SPECT.Methods: A total of 44 de novo drug-naïve PD patients [ with normal cognition and 17 with MCI ], 23 MCI patients and 23 individuals with subjective cognitive impairment were recruited at the Clinical Neurology Unit of Santa Chiara hospital. They were assessed by a wide neuropsychological battery, including Visual Search (...) Test measuring selective attention. Performances among groups were compared by non-parametric tests. Further, Spearman's rank correlations were performed to explore the association between neuropsychological variables and 123I-FP-CIT SPECT data in PD subgroup.Results: PD-MCI patients performed worse on VST than patients with PD-NC, patients with MCI and individuals with SCI. The performance of PD-MCI patients on VST significantly correlated with caudate nucleus 123I-FP-CIT SPECT uptake, whereas a negative correlation between such test and 123I-FP-CIT SPECT uptake in the left putamen was found in PD-NC patients.Conclusions: We suggest that selective attention deficit might be a trigger of cognitive decay in de novo PD-MCI patients. The VST should be routinely used to detect attentional deficits in hospital clinical practice, in the light of its closely association with dopamine depletion of basal ganglia in mildly impaired PD patients. Pour bien comprendre ce courant de pensée, il faut voir le rationalisme comme une tentative double, touchant à la fois la science et la philosophie. D’une part, le rationalisme tente de saisir la dimension culturelle de la science, d’autres part, il vise à élaborer une conception nouvelle, plus ouverte, de la raison philosophique. L’ouvrage s’intéresse également à l’influence exercée par (...) la pensée italienne sur la pensée française contemporaine. Il montre également comment le rationalisme en Italie s’est ouvert à l’historicisme sans pour autant fragiliser la structure même de la raison. This paper questions different conceptions of Medical Humanities in order to provide a clearer understanding of what they are and why they matter. Building upon former attempts, we defend a conception of Medical Humanities as a humanistic problem-based approach to medicine aiming at influencing its nature and practice. In particular, we discuss three main conceptual issues regarding the overall nature of this discipline: (i) a problem-driven approach to Medical Humanities; (ii) the need for an integration of Medical Humanities into medicine; (...) Martin Heidegger postula a recuperação da potência criativa da palavra como sendo a tarefa fundamental da fenomenologia, identificando na poesia a conservação dessa forma privilegiada de relação com a linguagem. O presente trabalho trata da linguagem como via de acesso às esferas primordiais do ser através de uma interpretação das experiências místicas como ocasiões para o rompimento do automatismo linguístico que impede ver na palavra algo além de um mero instrumento para a transmissão de informações. A partir das reflexões sobre (...) o símbolo desenvolvidas por pensadores como Gaston Bachelard, Paul Ricoeur, Mircea Eliade e Gershom Scholem, propomos uma conceituação fenomenológica do símbolo como entidade de sentido aberto, cuja origem remete ao positivamente indeterminado que constitui o “objeto” das experiências místicas. Positivamente indeterminado é a nossa expressão para dizer o divino, interpretando-o como fonte de todo sentido que é, por isso mesmo, ausente de qualquer sentido determinado, não por uma falta, mas por um excesso de significação. O excesso de significação se apresenta, então, como a chave para uma forma de relacionamento com a palavra concebida como força de criação. (shrink)

Exceção, elucidaremos o conceito de estado de exceção e o processo que, ao transformá-lo em prática permanente, marcou o ingresso definitivo do Estado na lógica do terrorismo. Advance care planning (ACP) is considered a pivotal aid in the decision‐making process, especially for many people living with dementia, who inevitably will lose the capacity to make decisions at the end of life. In Italy, ACP has been recently regulated by law 219/2017, leading to the investigation of how physicians deal with ACP in dementia. In order to comprehend the perception of physicians who provide care for patients with dementia regarding ACP and to describe their difficulties and needs, this (...) study presents the results of a qualitative research study performed within a local health authority in northern Italy. Data were collected by semi‐structured interviews with 11 physicians dealing with patients with dementia and were analysed using interpretative phenomenological analysis. Four superordinate themes were identified, describing both the difficulties and challenges perceived by physicians in performing shared ACP in their daily activities. This study demonstrates that physicians experience troubles synchronizing the time of shared ACP with the more rapid development of dementia. Results also suggest that further training about communication on ACP is urgently required to ease the implementation of shared ACP. A broader involvement of institutions and other professionals is also important in spreading ACP among healthcare institutions. (shrink)

De Luca uses classical thought to clarify contemporary and foundational issues in political theory.

The present study aimed to verify the quarantine’s effects during a serious viral outbreak on the cardiovascular and performance associated with the Yo-Yo test in a sample of professional soccer players. 20 high-level soccer players participated in this study. The intermittent Yo-Yo test was performed pre- and post- COVID-19 quarantine in a random order. During each test, the soccer players’ running performance outcomes were monitored using a portable 5-Hz GPS with a 100 Hz accelerometer and a paired t-test was conducted (...) at a p-value of ≤ 0.05. The main results demonstrated significant differences between pre- versus post-COVID-19 quarantine in the following variables: relative distance, maximal speed, acceleration, deceleration, sprints > 19 km/h [0.8 % >0.5 %], and in high intensity running distance [16.48 m > 0.827 m]. We concluded that COVID-19-related restrictions and quarantine COVID-19 demonstrated adverse effects on professional soccer players’ Yo-Yo tests performance. (shrink)

À côté de cette signification ordinaire, les luttes handies se sont emparées du verbe dévalider pour signifier deux aspects contradictoires mais complémentaires. D’un côté, les mondes dévalidés sont les mondes (...) que prépare la lutte anti-validiste, c’est-à-dire une société sans validisme (dé-validé en ce sens qu’il n’est plus rapporté à la norme valide). De l’autre, les mondes dévalidés désignent les mondes que prépare le capitalisme extractiviste, c’est-à-dire des mondes où plus personne ne peut être dit valide (dévalidé en ce sens que tout le monde, bien qu’à des degrés différents, y voit ses capacités déplacées, par exemple en raison de la toxicité de l’atmosphère). Les mouvements pour la justice handie se situent au point de tension entre d’un côté une lutte pour une dévalidation libératrice (contre le validisme qui rend le handicap invivable) et de l’autre une lutte contre la dévalidation forcée (contre l’extractivisme qui exploite la débilitation des vies). Background: Seeking consent for genetic and genomic research can be challenging, particularly in populations with low literacy levels, and in emergency situations. All of these factors were relevant to the MalariaGEN study of genetic factors influencing immune responses to malaria in northern rural Ghana. This study sought to identify issues arising in practice during the enrolment of paediatric cases with severe malaria and matched healthy controls into the MalariaGEN study. Methods: The study used a rapid assessment incorporating multiple qualitative methods (...) including in depth interviews, focus group discussions and observations of consent processes. Differences between verbal information provided during community engagement processes, and consent processes during the enrolment of cases and controls were identified, as well as the factors influencing the tailoring of such information. Results: MalariaGEN participants and field staff seeking consent were generally satisfied with their understanding of the project and were familiar with aspects of the study relating to malaria. Some genetic aspects of the study were also well understood. Participants and staff seeking consent were less aware of the methodologies employed during genomic research and their implications, such as the breadth of data generated and the potential for future secondary research.Moreover, trust in and previous experience with the Navrongo Health Research Centre which was conducting the research influenced beliefs about the benefits of participating in the MalariaGEN study and subsequent decision-making about research participation. Conclusions: It is important to recognise that some aspects of complex genomic research may be of less interest to and less well understood by research participants and that such gaps in understanding may not be entirely addressed by best practice in the design and conduct of consent processes. In such circumstances consideration needs to be given to additional protections for participants that may need to be implemented in such research, and how best to provide such protections.Capacity building for research ethics committees with limited familiarity with genetic and genomic research, and appropriate engagement with communities to elicit opinions of the ethical issues arising and acceptability of downstream uses of genome wide association data are likely to be important. Esta obra es una reivindicación de la figura de Giner de los Ríos como uno de los más grandes hitos de la historia del pensamiento español al anticipar teorías filosóficas, científicas, políticas, morales, legales que se desarrollarán en el siglo XX, ocupando en todos los casos el hombre y lo humano un puesto central: un hombre, en torno al cual se contextualiza el mundo ético del krausismo, en el que cada uno de nosotros está abocado al bien como deber vital. (...) Concilia la autonomía del ciudadano con valores universales, desarrollando todo un sistema humanista racional, liberal y abierto a lo social. Otro rasgo de su pensamiento es su modernidad: su objetivo es reformar las estructuras del país a través de la educación: el aspecto más conocido de su obra y cuyo punto de partida es su antropología. El resultado será la transformación social (en un país tan necesitado de ella), un cambio de la persona no por la razón técnica sino moral: individuos libres en una sociedad que la garantiza. Con ello busca la creación de un pueblo adulto por medio de la aplicación de su concepto de Humanidad. Y así, en su búsqueda de una nación de hombres justos y virtuosos nos puso en contacto con el pensamiento europeo: introdujo en nuestro país el krausopositivismo y los avances de la nueva ciencia que se desarrolla desde la segunda mitad del siglo XIX, limitados por la moral. La Institución Libre de Enseñanza sería el intento de realizar la transformación del hombre, según unos valores de plena vigencia en el presente. Giner fue, en definitiva, el impulsor del èthos de la civilidad, cuyo núcleo central reside en el imperativo categórico kantiano: hacer el bien por el bien mismo. (shrink)

Punishment and forgiveness are two very different responses to a moral transgression that both have been argued to restore perceptions of moral order within an organization. Unfortunately, it is currently unclear what motivates organizational actors to punish or forgive a norm transgressor. We build on social cognitive theory to argue that punishment and forgiveness of a transgressor are both rooted in self-regulatory processes. Specifically, we argue that organizational actors are more likely to respond to intentional transgressions with punishment, and to (...) unintentional transgressions with forgiveness. However, these effects of transgressor intentionality should be found in particular among actors for whom moral identity is central. We find support for these predictions in a laboratory experiment and a field study among organizational leaders. By simultaneously studying punishment and forgiveness in organizational settings, we provide crucial insight in their shared motivational bases, as well as into important differences between the two. Leg rigidity is associated with frequent falls in people with Parkinson’s disease, suggesting a potential role in functional balance and gait impairments. Changes in the neural state due to secondary tasks, e.g., activation maneuvers, can exacerbate rigidity, possibly increasing the risk of falls. However, the subjective interpretation and coarse classification of the standard clinical rigidity scale has prohibited the systematic, objective assessment of resting and activated leg rigidity. The pendulum test is an objective diagnostic method that we hypothesized would be (...) sensitive enough to characterize resting and activated leg rigidity. We recorded kinematic data and electromyographic signals from rectus femoris and biceps femoris during the pendulum test in 15 individuals with PD, spanning a range of leg rigidity severity. From the recorded data of leg swing kinematics, we measured biomechanical outcomes including first swing excursion, first extension peak, number and duration of the oscillations, resting angle, relaxation index, maximum and minimum angular velocity. We examined associations between biomechanical outcomes and clinical leg rigidity score. We evaluated the effect of increasing rigidity through activation maneuvers on biomechanical outcomes. Finally, we assessed whether either biomechanical outcomes or changes in outcomes with activation were associated with a fall history. Our results suggest that the biomechanical assessment of the pendulum test can objectively quantify parkinsonian leg rigidity. We found that the presence of high rigidity during clinical exam significantly impacted biomechanical outcomes, i.e., first extension peak, number of oscillations, relaxation index, and maximum angular velocity. No differences in the effect of activation maneuvers between groups with clinically assessed low rigidity were observed, suggesting that activated rigidity may be independent of resting rigidity and should be scored as independent variables. Moreover, we found that fall history was more common among people whose rigidity was increased with a secondary task, as measured by biomechanical outcomes. We conclude that different mechanisms contributing to resting and activated rigidity may play an important yet unexplored functional role in balance impairments. The pendulum test may contribute to a better understanding of fundamental mechanisms underlying motor symptoms in PD, evaluating the efficacy of treatments, and predicting the risk of falls. From past research it is well known that social exclusion has detrimental consequences for mental health. To deal with these adverse effects, socially excluded individuals frequently turn to other humans for emotional support. While chatbots can elicit social and emotional responses on the part of the human interlocutor, their effectiveness in the context of social exclusion has not been investigated. In the present study, we examined whether an empathic chatbot can serve as a buffer against the adverse effects of social (...) ostracism. After experiencing exclusion on social media, participants were randomly assigned to either talk with an empathetic chatbot about it (e.g., “I'm sorry that this happened to you”) or a control condition where their responses were merely acknowledged (e.g., “Thank you for your feedback”). Replicating previous research, results revealed that experiences of social exclusion dampened the mood of participants. Interacting with an empathetic chatbot, however, appeared to have a mitigating impact. In particular, participants in the chatbot intervention condition reported higher mood than those in the control condition. Theoretical, methodological, and practical implications, as well as directions for future research are discussed. (shrink)

This collection addresses two different audiences: 1) historians and philosophers of the life sciences reflecting on collaborations across disciplines, especially as regards defining and addressing Grand Challenges; 2) researchers and other stakeholders involved in cross-disciplinary collaborations aimed at tackling Grand Challenges in the life and medical sciences. The essays collected here offer ideas and resources both for the study and for the practice of goal-driven cross-disciplinary research in the life and medical sciences. We organise this introduction in three sections. The (...) first section provides some background and context. The second motivates our take on this topic and then outlines the central ideas of each paper. The third section highlights the specificity and significance of this approach by considering: a) how this collection departs from existing literature on inter- and trans-disciplinarity, b) what is characteristic about this approach, and c) what role this suggests for the history and philosophy of the life sciences in addressing Grand Challenges. Hume’s Principle states that the cardinal number of the concept F is identical with the cardinal number of G if and only if F and G can be put into one-to-one correspondence. The Schwartzkopff-Rosen Principle is a modification of HP in terms of metaphysical grounding: it states that if the number of F is identical with the number of G, then this identity is grounded by the fact that F and G can be paired one-to-one, 353–373, 2011, 362). HP is (...) central to the neo-logicist program in the philosophy of mathematics ; in this paper we submit that, even if the neo-logicists wish to venture into the metaphysics of grounding, they can avoid the SR Principle. In Section 1 we introduce neo-logicism. In Sections 2 and 3 we examine the SR Principle. We then formulate an account of arithmetical facts which does not rest on the SR Principle; we finally argue that the neo-logicists should avoid the SR Principle in favour of this alternative proposal. The idea of personalised medicine (PM) has gathered momentum recently, attracting funding and generating hopes as well as scepticism. As PM gives rise to differing interpretations, there have been several attempts to clarify the concept. In an influential paper published in this journal, Schleidgen and colleagues have proposed a precise and narrow definition of PM on the basis of a systematic literature review. Given that their conclusion is at odds with those of other recent attempts to understand PM, we consider (...) whether their systematic review gives them an edge over competing interpretations. We have found some methodological weaknesses and questionable assumptions in Schleidgen and colleagues’ attempt to provide a more specific definition of PM. Our perplexities concern the lack of criteria for assessing the epistemic strength of the definitions that they consider, as well as the logical principles used to extract a more precise definition, the narrowness of the pool from which they have drawn their empirical data, and finally their overlooking the fact that definitions depend on the context of use. We are also worried that their ethical assumption that only patients’ interests are legitimate is too simplistic and drives all other stakeholders’ interests—including those that are justifiable—underground, thus compromising any hope of a transparent and fair negotiation among a plurality of actors and interests. As an alternative to the shortcomings of attempting a semantic disciplining of the concept we propose a pragmatic approach. Rather than considering PM to be a scientific concept in need of precise demarcation, we look at it as an open and negotiable concept used in a variety of contexts including at the level of orienting research goals and policy objectives. We believe that since PM is still more an ideal than an achieved reality, a plurality of visions is to be expected and we need to pay attention to the people, reasons and interests behind these alternative conceptions. In other words, the logic and politics of PM cannot be disentangled and disagreements need to be tackled addressing the normative and strategic conflicts behind them. (shrink)

Both a significant body of literature and the case study presented here show that digital knowledge repositories struggle to attract the needed level of data and knowledge contribution that they need to be successful. This happens also to high profile and prestigious initiatives. The paper argues that the reluctance of researchers to contribute can only be understood in light of the highly competitive context in which research careers need to be built nowadays and how this affects researchers’ quality of life. (...) Competition and managerialism limit the discretion of researchers in sharing their results and in donating their working time. A growing corpus of research shows that academic researchers are increasingly overworked and highly stressed. This corroborates the point that the room for undertaking additional tasks with future and uncertain benefits is very limited. The paper thus recommends that promoters of digital knowledge repositories focus on the needs of the researchers who are expected to contribute their knowledge. In order to treat them fairly and to ensure the success of the repositories, knowledge sharing needs to be rewarded so as to improve the working conditions of contributors. In order to help implementing this researcher-centred approach, the paper proposes the idea of expediential trust: rewards for contributing should be such that rational, self-interested researchers would freely decide to contribute their knowledge and effort trusting that this would make them better off. In this paper, the ontological, terminological, epistemological, and ethical aspects of omission are considered in a coherent and balanced framework, based on the idea that there are omissions which are actions and omissions which are non-actions. In particular, we suggest that the approach to causation which best deals with omission is Mackie's INUS conditional proposal. We argue that omissions are determined partly by the ontological conditional structure of reality, and partly by the interests, beliefs, and values of observers. The final (...) upshot is that moral judgments involved in cases of omissions cannot be grounded on, but are the ground for judgments about what INUS conditions count as omissions. (shrink)
